When chronic kidney disease reaches an advanced stage, dangerous levels of fluid, electrolytes and wastes can build up in the body. Thus, disrupting the blood vessels of the retinal system, which causes vision loss. The anomalies in the vascular structure of the retina such as morphological changes in the form of vessel shape, tortuosity or retinal lesions. Thus, a study to establish the correlation between retinal blood vessel tortuosity and chronic kidney disease is conducted. This study proposes an automated method for the analysis of retinal vessel network and measures the tortuosity regarding arteries and veins in fundus images. The fundus images are extracted and measured by establishing quadrant-based tortuosity measurement in which the extracted skeletonised blood vessel is divided into four quadrants. The quantitative method includes structural mapping of the vessel network and quantification method based on the measurement of vessel tortuosity. This system uses an automated retinal blood vessel approach improved multi-scale line detection for optimising and segmenting blood vessels in fundus images. The quantification framework uses the curvature-based method to measure the tortuosity of the blood vessel in which modified curvature-based method that applies trigonometric identities to curvature function is established. Tortuosity measurement is implemented on a total of 48 fundus images of patients who were diagnosed with CKD and diabetic retinopathy (DR). The fundus images are obtained from DRIVE, STARE and clinical result databases. Upon analysing nine subjects of chronic kidney disease fundus image, it is found that 8 of them to be tortuous with tortuosity index between 0.65905 to 0.71785. The suggested algorithm also produced a tortuosity index trend indicating a substantial prevalence of tortuosity in fundus images of patients with chronic kidney disease. Hence, implying that there is an association between retinal blood vessel tortuosity and chronic kidney disease. The significant contribution of this study includes the development of a collection of automatic methods for vessel segmentation and establishing an automated process for calculating the morphological property of retinal vessels specifically tortuosity to aid in the diagnosis of CKD and developing an automated system for explaining the presence of tortuosity in the blood vessels to differentiate between normal subjects and those with CKD. 2023-05-03T16:46:50Z 2023-05-03T16:46:50Z 2020-09 https://irepository.uniten.edu.my/handle/123456789/21397 en application/pdf
