It commonly occurs on the lower lip. However, mucous extravasation cyst involving ventral surface of the tongue is uncommonly found in paediatric patient. Case Report: This report was highlighted the successful management of a rare case of mucous extravasation cyst involving the tip of ventral surface of the tongue in a 11- year-old boy. The clinical examination revealed the presence of a soft tissue nodule on the tip of ventral surface of the tongue which was pink pale in colour compared to the oral mucosa measuring approximately 5 x 7 mm with a pedunculated base, firm consistency, clearly defined border and fix in position. Discussion: The aetiology of the above condition was due to trauma that occurred because of the overlapping between primary and permanent anterior teeth of mandibular. Surgical removal of mucous extravasation cyst can be challenging, especially in children. Nonpharmacological behaviour management is essential in paediatric patients. A child with an oral pathology is anxious and susceptible to behaviour problems. For this case, several non-pharmacological behaviour managements were applied throughout this procedure such as preparatory information, enhance control, behaviour shaping and positive reinforcement to ensure the treatment done smoothly and successfully. Conclusion: Diagnosis and management of mucous extravasation cyst in children are challenging. This case report revealed the clinical characteristics, histological features, differential diagnosis and the treatment option in order to aid decision- making in daily clinical practice.
